Therefore it prompted us to investigate whether epigenetic modifications play a role in initiation and/or perpetuation of atrial fibrillation. Methods Atrial DNA hypermethylation and histone modifications were analyzed using methyl DNA immunoprecipitation, promoter capture chip and ChIP analysis respectively. Results Using methyl DNA immunoprecipitation and promoter capture chip assays we identified significant hypermethylation at promoter region of WT1 gene in patients presenting with persistent atrial fibrillation compared to normal sinus rhythm controls. Follow up ChIP analysis confirmed the histone modifications that are regarded as ‘epigenetic signatures’ of DNA methylation at centromeric portion of WT1 gene (centromere cluster). Conclusions These results reveal epigenetic deregulation of WT1 gene as a novel mechanism that may be related with atrial fibrillation development irrespective of other chronic cardiovascular risk factors known to be associated with atrial fibrillation. Keywords - Atrial Fibrillation - DNA Methylation - Gene Silencing - Histone Modification - WT1 Gene Background Atrial fibrillation (AF) is the most common supraventricular tachyarrhythmia and its incidence increases by age and accounts for around 17% of all patients referred for electrical cardioversion [1]. Mechanisms that predispose atria to perpetuation of AF include altered expression of ion channels with conduction abnormalities involving shortening of action potential duration (APD) and conduction velocity [2]. Studies have demonstrated that alterations of channel activity may be related with changes in mRNA and/or protein expression or directly related with kinetic properties of channel proteins [2]. However studies focusing on molecular genetic conditions that influence atrial fibrillation are still limited. The Wilms’ tumor gene (WT1) is located at chromosome band 11p13 and is named after the kidney cancer found in children that is associated with its mutation [3]. WT1 encodes a zinc finger protein with highly regulated expression during embryonic development [4]. In adult tissue its expression is restricted to lymphoid tissues [5], however recent studies suggest that WT1 is widely expressed throughout normal human tissues [6]. In vitro studies have demonstrated that WT1 binds a much larger set of target genes than previously thought [7]. The broad range of WT1 functions include development of respectively hematopoietic system [8], kidney [9], retina [10], nerve [11], heart [12] as well as regulation of cell growth [13] plasticity [14] proliferation [15], apoptosis [16] and p53 tumor suppressor gene transcription [17]. In these contexts the WT1 gene acts as a multifunctional transcription factor with tumor suppressor and oncogenic properties depending on its cellular expression. Furthermore, recent studies suggest that WT1 is implicated in regulation of cardiac response upon stress conditions such as myocardial infarction [18]. In respect to its regulatory influence on cellular growth, studies have linked WT1 expression with transcriptional repression mechanisms through histone deacetylases (HDACs) which promote chromatin remodeling and transcriptional repression [19]. These studies suggest that WT1 mutations may predispose heart to reduction of cardiomyocyte proliferation capabilities as observed in some neonatal cardiomyopathies [20]. However as WT1 is not typically expressed in adult cardiomyocytes it is unclear whether WT1 could be implicated in other regulatory pathways regulating cellular processes involving proliferation or gene expression. On the contrary epigenetic regulatory mechanisms which include DNA methylation are regarded as alternative regulatory mechanisms that affect chromatin structure and control gene transcription irrespective of other genetic alternatives [21]. It has been well established that hypermethylation of CpG islands present at promoter regions of genes relates with reduction of gene transcription in various types of cancers [22]. Even more studies have suggested that epigenetic modifications such as DNA methylation may be related with gene silencing associated with myocardial disease progression [23]. Therefore gene silencing at the level of DNA methylation has emerged as a key mechanism involved in pathogenesis of variety of diseases including heart failure [24], dilated cardiomyopathy (DCM) [25] respiratory diseases such as asthma [26] but also autoimmune disorders such as systemic lupus erythematosus [27] and rheumatoid arthritis [28]. These studies have indicated that there are substantial differences in genomic DNA methylation patterns between diseased patients compared to healthy individuals. Furthermore they suggest that hypermethylation observed at promoter regions of particular genes may be implicated as a causal mechanism or may play a supportive role in disease pathogenesis by aggravating deleterious effects linked with genetic mutations. Adding atrial fibrillation and heart failure together as common expressions of cardiovascular disease it might be expected that atrial DNA hypermethylation may play a role in pathogenesis or development of this arrhythmia.
